State and Federal laws strictly define the difference between employees and contractors. These definitions in turn describe the rights and responsibilities of the District and the party delivering the service to the District.
In fulfilling the District’s obligations, an Independent Contractor Form, Request for Taxpayer Identification Number and Certification Form W-9 must be on file with Business Services for any individual who provides services for payment, when payment does not occur through the payroll process. Completed forms must be on file before any commitment is made for services rendered.
WIAA certified athletic officials must have a completed a W-9 and Direct Deposit form on file with Business Services.
The Independent Contractor Form requires responses to a series of questions that distinguish individuals as independent contractors rather than employees.
The Request for Taxpayer Identification Number and Certification Form W-9 is used by the District to confirm the Taxpayer Identification Number used to submit informational returns to the IRS.
