When third parties are providing services on School District of Holmen sites, they may request a Certificate of Property Insurance to verify the District has property insurance. In order to request a Certificate of Property Insurance, send your request to ESOV@asugroup.com. Certain information is needed in order to process your Certificate such as, but not limited to:

• School District of Holmen name and contact information
• Whether the District needs to be endorsed as a Loss Payee
• A description of the property/equipment the certificate is covering
