In the event an employee is injured at work, the employee must go to the Main Office at the building to report the injury to his/her Supervisor and request an injury packet. Please work with the Supervisor to complete the information provided in the packet. Return the completed packet to the District’s Benefits Specialist as soon as possible.
Additionally, in the event the employee intends to seek medical treatment or the injury status changes, the employee must notify his/her Supervisor immediately. Please return all Physician Recommendation or Status Reports to the Benefits Specialist as soon as possible.
